Case 48: This is a good example of finesse rhinoplasty where subtle changes can make for a very nice and meaningful difference. By the next day, I was looking more like a monster and didn't want to see anyone. Many different types of breast lifts are now available in conjunction with a breast reduction, including lifts just around the areola (periareolar), vertical/lollipop mastopexies (incision around the areola and down to the breast fold, and full breast lifts with an anchor incision. For patients who have very loose skin in the armpit or back, axillary or bra lifts are also often combined with breast reductions.

Since the mid-1990s, the fifth generation of silicone-gel breast implant is made of a high-strength, highly cohesive silicone gel that mostly eliminates the occurrences of filler leakage (“silicone gel bleed”) and of the migration of the silicone filler from the implant pocket to elsewhere in the woman's body. These implants are commonly referred to as "gummy bear breast implants" for their firm, pliant consistency, which is similar to gummy candies. The studies Experience with Anatomical Soft Cohesive Silicone gel Prosthesis in Cosmetic and Reconstructive Breast Implant Surgery (2004) and Cohesive Silicone gel Breast Implants in Aesthetic and Reconstructive Breast Surgery (2005) reported low incidence-rates of capsular contracture and of device-shell rupture; and greater rates of improved medical-safety and technical-efficacy than that of early generation breast implant devices.[94][95][96]

The ARTOURA™ Breast Tissue Expander or CONTOUR PROFILE® Breast Tissue Expander can be utilized for breast reconstruction after mastectomy, correction of an underdeveloped breast, scar revision, and tissue defect procedures. The expander is intended for temporary subcutaneous or submuscular implantation and is not intended for use beyond six months. Do not use the ARTOURA Tissue Expander nor CONTOUR PROFILE® Tissue Expander in patients where an MRI may be needed. The device could be moved by the MRI causing pain or displacement, potentially resulting in a revision surgery. The incidence of extrusion of the expander has been shown to increase when the expander has been placed in injured areas.


For the second part....it depends where your butt is starting at. There is a point in which there is too much pressure in the butt that no more fat can really be injected. Pressure kills the fat cells (which is why you cannot sit on the butt for a few weeks after surgery). It also means that we cannot just keep injecting if the gluteus has fully expanded and is not able to take any more. If you are starting with a small butt.....there is a smaller limit. If you are starting with a medium sized butt.....there is a bigger limit. I like to use power assisted "vibration expansion filling" to really push the boundaries of how much fat can be injected into the buttocks. This helps to expand the buttocks on the table and get more fat into more stubborn areas. Typically, even for smaller girls, I will inject at least 1000ml per side (and up to 1600ml for larger girls). After this, as you have probably read, 30-50% of the fat will either reabsorb or "not take". However, with the removal of a large amount of fat around your midsection combined with careful technique and contouring of the butt and hips, we can make a very large difference in your overall proportion. Best of luck to you!
Sometimes insurance pays for a rhinoplasty, but it depends on the insurance policy. Before scheduling surgery, your doctor's office will help you get prior written authorization from your insurance company. Although this isn't a guarantee of coverage, it's the only way to confirm that rhinoplasty is a covered benefit. Sometimes insurance will pay for a part of a nasal surgery, but not other parts. By and large, there is no 'perfect type' of breast implant. What looked stunning on one of your best friends doesn't necessarily mean that the same implant will be the best choice for you. This is why we recommend face-to-face consultations; an honest discussion with an experienced, knowledgeable plastic surgeon about your desired enhancement is a must. And you'll want to consider your surgeon's opinion and recommendations based on your existing body type and physical assessment.

2. You buttock's structural anatomy is also an important factor.  A V-shaped buttock typically requires more fat transfer to achieve a satisfactory improvement than an A-shaped buttock.  Transferred fat cells need to grow their own blood supply from the surrounding buttock tissues in order to survive.  In order for this to happen, the transferred fat cells need to be gently interwoven between your buttocks' living cells to survive.  Therefore, each person's buttocks can only receive a certain amount of fat, and exceeding that amount will not be effective.  A generous amount of fat is typically transferred at the time of surgery, knowing that some of the fat will go away during the recovery period.
The woman wanting a lift is usually slightly different. She had breasts she was happy with before, she had loving kids who she may or may not have breastfed, exercised and had a good life. They come wanting to restore the youthful breast they once they had(or better), they hate that it is slightly more deflated than before and it's slightly more south then they'd like. The formal name of this operation is "Mastopexy" and that's why you hear terms like "Mastopexy augmentation" because often in order to restore the youthfulness of the breast you not only need to reposition the nipple/areolar complex "up" with a mastopexy, you also need to provide some additional volume with an implant, hence augmentation as well. The discussion with implant also is entirely different discussion all together, but this highlights the primary difference in the reduction and a lift.

Of course, the breast reduction vs. breast lift question is not really cut and dry because every woman is unique. You may be unsure of what volume you would like with the breasts in a more favorable position. Breasts may be different sizes (asymmetric). The position of the nipple may be out of proportion to the amount of breast sag. Breasts may be ‘deflated’ after pregnancy. You may have heard that breast implants are the only reliable way to lift breasts. These questions can easily be addressed at the time of your consult.
Fat transfer breast augmentation has a few important limitations. First, fat transfer is typically limited to approximately one cup size increase in breast size. Second, the transferred fat may be absorbed again by the body. Third, it is more expensive because of the extra time involved in harvesting and processing donor fat as well as possible need for a special vacuum bra device. This option is best reserved for women who desire a modest increase in size without using an implant.

Breast reconstruction with fat grafting: If you have breast defects following lumpectomy for breast cancer, fat grafting is an effective method for filling in these defects. Fat grafting is also an option for total breast reconstruction following mastectomy; however, to achieve sufficient breast volume, this is usually a multistage process that requires at least two to four sequential fat grafting procedures.
Women with ruptured silicone gel implants also need to factor in downtime when asking how much do breast implants cost. Whether you take time off work, hire a babysitter for your kids, or make other accommodations while you recover from surgery, you need to factor in these costs. Dr. Jane Rowley, a board-certified plastic surgeon in Lubbock, Texas, explains the difference between removal of silicone gel implants and the IDEAL IMPLANT, “there’s a big difference between removing a ruptured IDEAL IMPLANT and a ruptured silicone gel implant. A ruptured IDEAL IMPLANT can be removed easily with a local anesthetic. A silicone gel implant, if they’re not ruptured, can come out that easy. But if they are ruptured, most of the time its stuck, and it bleeds, and you have to put drains in. So it’s not an easy recovery, it’s not an easy surgery. With an IDEAL IMPLANT I can almost guarantee them ahead of time, if your implant is leaking it will be easy to remove and replace, you will be back to work in a day or two at the most. With a silicone gel implant I’ll say, ‘I don’t know what your recovery is going to be, it might come out easily, it might not come out easily. You might need to take week off work, you might have to take two days off work.’”
After reviewing the medical data, the U.S. Food and Drug Administration concluded that TDA-induced breast cancer was an infinitesimal health-risk to women with breast implants, and did not justify legally requiring physicians to explain the matter to their patients. In the event, polyurethane-coated breast implants remain in plastic surgery practice in Europe and in South America; and no manufacturer has sought FDA approval for medical sales of such breast implants in the U.S.[93]
Case 78: A combination approach was used here to create a really meaningful but completely natural transformation. Otoplasty made prominent ears all but disappear from the field of view and no longer distract or draw attention. At the same time, rhinoplasty, chin augmentation, and neck liposuction accomplished were able to remove a nasal hump and overprojection while improving chin and neck laxity to achieve a nice overall balance.
The human body's immune response to a surgically installed foreign object—breast implant, cardiac pacemaker, orthopedic prosthesis—is to encapsulate it with scar tissue capsules of tightly woven collagen fibers, in order to maintain the integrity of the body by isolating the foreign object, and so tolerate its presence. Capsular contracture—which should be distinguished from normal capsular tissue—occurs when the collagen-fiber capsule thickens and compresses the breast implant; it is a painful complication that might distort either the breast implant, or the breast, or both.

Both saline-filled breast implants and silicone-filled implants have an outer shell composed of silicone elastomer. This shell is basically a flexible envelope that contains the implant filling. In the case of some anatomically shaped implants, the shell also gives the implants shape. Some models of implants have a "double lumen." This is an elastomer envelope inside of another elastomer envelope (sort of like double-bagging your groceries) which may reduce the risk of implant rupture.
Fat transfer, is a safe, natural, minimally invasive procedure. Fat from one area of your body is transferred to another part of your body –hence the name, “fat transfer”. How it works is some parts of your body may have excess fat that can be utilized by areas that have lost their fullness or in areas that never had it in the first place. Some of these areas can be thin lips, calves, cheeks, breasts, hands, pock marks, buttocks, scars, and marks from previous cosmetic procedures. Many choose to have a fat transfer procedure with a combination of an Breast Lift to reduce sagging breasts.
MENTOR® MemoryGel® Breast Implants, MENTOR® MemoryShape® Breast Implants, and MENTOR® Saline-filled Breast Implants are indicated for breast augmentation in women (at least 22 years old for MemoryGel® Implants and MemoryShape® Implants, and 18 years old for Saline Implants) or for breast reconstruction. The breast cancer studies Cancer in the Augmented Breast: Diagnosis and Prognosis (1993) and Breast Cancer after Augmentation Mammoplasty (2001) of women with breast implant prostheses reported no significant differences in disease-stage at the time of the diagnosis of cancer; prognoses are similar in both groups of women, with augmented patients at a lower risk for subsequent cancer recurrence or death.[103][104] Conversely, the use of implants for breast reconstruction after breast cancer mastectomy appears to have no negative effect upon the incidence of cancer-related death.[105] That patients with breast implants are more often diagnosed with palpable—but not larger—tumors indicates that equal-sized tumors might be more readily palpated in augmented patients, which might compensate for the impaired mammogram images.[106] The ready palpability of the breast-cancer tumor(s) is consequent to breast tissue thinning by compression, innately in smaller breasts a priori (because they have lesser tissue volumes), and that the implant serves as a radio-opaque base against which a cancerous tumor can be differentiated.[107]
For women who experience breast sagging, I would recommend a breast lift; not breast reduction surgery. Breast reduction can provide a more modest breast size by removing tissue and skin from the breasts; however, a breast lift is needed to achieve a perkier, higher-positioned breast contour. It’s very common for patients to combine their breast reduction surgery with a breast lift to achieve more comprehensive breast enhancement results.
There are two main types of implants for you to consider: saline and silicone. Most people say the silicone implants feel and look more natural than saline, but they cost more. Silicone implants are typically about $1,000 more than saline, but if you’re interested in gummy bear implants—a type of silicone implant—expect to pay even more. The saline is administered via a process that ensures the implants remain sterile.

The United States FDA strongly encourages all physicians to report cases to PROFILE in an effort to better understand the role of breast implants in ALCL and the management of this disease.[74]
