For your case, I would decide what breast size you want to be. That will determine what size breast implant you need. The size the breast implant will determine also what type of breast lift you need. With a bigger implant, you may be able to get by with a donut breast lift or a circumperiareolar mastopexy. This may be the best way to start. If you still don’t get your desired breast shape and look you desire, you can always convert to a vertical or anchor mastopexy. Botox Breast Lift: There are more and more uses of Botox that researchers are finding as time goes on. A Botox Breast Lift is not an approved use of botox by the FDA. Thankfully, there’s a slew of solutions that have been proven to help with scarring, even completely removing the visual effect. One of the first solutions that may be offered is a steroid injection into the area, which helps with the discomfort and redness of scarring. Cortisone creams are another option that may help to reduce the size of the blemish. Targeted cryotherapy, similar to the type of therapy used to remove warts, may also reduce scarring by freezing the tissue away.
It all depends on the position of the nipple/areolar complex position. If there is significant loose skin, then the vertical incision is necessary to give you the ideal result. The length of the vertical scar is not long, only 4-5 cm from the bottom of the areola to the crease. This scar heals very well. If there is a lot of excess skin, then the anchor incision is necessary.
There is a basic principle that applies to most cash-pay health care procedures: the more reputable the surgeon, the more surgery will likely cost. A surgeon with considerable experience and recognition in the field of body sculpture can understandably command a higher fee. From your image, I would actually say that you don’t need the anchor incision breast lift, which requires the greatest scar burden, but it does appear that you would be best suited for a vertical breast lift with an incisional scar around the areola plus one down the lower pole of the breast. To achieve the degrees of lift you likely need, you need that vertical incision.
Liposuction of the hips is generally successful in creating a more contoured figure. This type of liposuction is usually performed on women, as men are not as susceptible to retaining fat deposits in this area. It's not unusual to see lumpy areas after the swelling starts to go down. This should improve after a few weeks, but wearing a compression garment can help prevent lumps. Less commonly, patients can have a hematoma (a temporary pooling of blood under the skin) or seroma (a pocket of fluid under the skin that requires draining by your doctor); changes in skin color or sensation; or scarring from thermal burning, either above or below the skin (usually an issue only with laser- and ultrasound-assisted lipo, if skin ports aren’t used). A circumareolar breast lift can enhance breasts with mild ptosis and results in almost no visible scarring; however, the lift of the nipple is limited to no more than 2 cm and the procedure often does not create a very perky breast as the breast tissue is effectively lifted. The scar can also be wide, it’s hard to keep the areola perfectly round, and this technique tends to flatten the breasts. An inverted-T lift can repackage breast tissue for a perkier appearance but the scar is an anchor configuration on the breast. The problem arises when you want an improved breast shape, better nipple position, and minimal scarring. Traditional techniques make you choose form over aesthetics, and for some women, this option is less than desirable.
The moon-shape (crescent) lift involves a scar hidden along the upper half of the areola border. The donut (circumareolar) lift involves a scar hidden around the entire circumference of the areola border. These scars camouflage very well where the darkly pigmented skin of the areola meets the lighter skin of the rest of the breast. These lifts work well when the nipple is only slightly downward pointing but positioned relatively well on the chest wall. One of the disadvantages of these lifts is that they can only raise the areola up to a maximum of about 2 cm. They also tend to flatten the breast shape and are not ideal when there is saggy breast tissue in the lower pole. Therefore, based on the low position of your nipple areola complex and the amount of breast tissue laxity you have, you would not be a good candidate for either of these lifts.

During the course of a woman’s life, the breasts change in appearance and oftentimes sag. Don’t worry, this is NOT an uncommon condition. In fact, the clinical term for female breasts that sag is called ‘breast ptosis.’ There are many causes of breast ptosis including pregnancy, cigarette smoking, high body mass, gravity and the natural aging process. There are also different degrees of ptosis as shown in the illustration below. Fortunately, breast ptosis can easily be treated with breast lift surgery.
Recovery also involves unique expenses. Compression garments are very important in liposuction recovery, aiding the healing process and helping the skin conform to your refined contours. I usually provide one free garment. Subsequent garments can cost between $50 and $100. During your recovery, you will be prescribed pain medication to ensure your comfort. If you are looking for the best looking scar, or the smallest scar, that would be the Benelli. This is eventually a ‘hidden scar’ as it will eventually fade into the nipple line. Most patients are also happy with the Vertical Scar as the scar is on the underside of the breast, and not exposed horizontally on the chest wall. In my opinion, the least attractive scar is the Inverted T technique (Wise Pattern), which is really two scars.
With this procedure, a surgeon creates an incision on or around the perimeter of the areola and later a second outer incision to remove the “donut” of skin around it. The nipple is sutured back to the breast skin, tightening it in the process. This method provides more lift than the crescent, but again only a relatively small amount of sagging can be repaired.
